To register your pregnancy with a midwife please use the online booking form below.
Mum’s to be can now book their pregnancy online using a secure form on the Cardiff & Vale University Health Board website.

A Midwife will contact you by telephone within 1-2 weeks of you sending your completed form to arrange your maternity booking appointment.This will include taking a full medical history to plan your maternity care.
If you have not had a response by 14 days – please phone 029 2074 5030 so that they can check your contact details
